Hello All!
My name is Samantha Conrad. I am the School Nurse at Blacklick Valley School District. Below are some resources you may find useful. If you have any questions or concerns, please contact me via email or phone.

Above is a link that reviews mandated health screenings such as height, weight, vision, and hearing. It also has a link to print out private physician physical forms.
**Physicals should be turned into the school on entry/Kindergarten, 6th grade, 11th grade.
**They must be done within one year prior to entry into the required grades.
**If there are difficulties (financial, finding a pediatrician, etc.) with getting a physical done or referrals for vision or hearing, please reach out to the nursing office.

Above is a link that reviews dental health and has a form to be filled out if the student sees a private dentist.
Students are mandated to see a dentist on entry/Kindergarten, 3rd, and 7th grade.
Important dates: May 8-9, 2023 the MOBILE DENTIST is coming to the Elementary (HS students will be bussed to the elementary to be seen). Permission forms will be sent home closer to date.

Above is a link of required immunizations for school aged children. If there is any difficulty regarding immunizations such as scheduling or cost, please contact the nurse.
If any form of exemption, please contact the nurse's office for documentation to sign.
***Copy of immunizations and or exemption form must be turned in by the first day of school.
